Fall 2010 Late Registration

Registration after May 8, 2010

 

STEPS TO FOLLOW FOR LATE REGISTRATION:

   1. Go to www.eayso.org and complete the pre- registration form  for each player.

2. Print a copy of this form/s.

3. Mail or deliver the forms and fee of $140.00per player to:

AYSO-Region 159

c/o UPS Store

12340 Seal Beach Blvd. Ste B-Box 313

Seal Beach, CA 90740

 

*Note:  First time players or players that did not participate in Region 159 last season must also mail a copy of a birth certificate or other official proof of age.

 

UPS Store is located in the Target Shopping Center/Seal Beach, next to Great Clips.

 

PLAYER WILL BE PLACED ON A WAITING LIST WITH NO GUARANTEE OF TEAM PLACEMENT.

U-5 PROGRAM IS BACK !!!
Players must be 4 years old by September 5th, 2010. Parent participation required.

Boys U19 team being formed for Fall 2010 Season.
